Pricing & Product Details (for Illinois Based Businesses)


  • Utilizes the VSP "Choice" Doctor Network
  • $10 copay for Exams & $25 copay for materials
  • Frequency- Eye Exams @ 12 months, Lenses @ 12 months, Frames @ 24 months
  • Option A is for groups of 10-99 enrolled employees, and when the vision plan is linked to a seperate Medical or Dental Insurance plan, OR if the employer is paying 75% or more of the Vision Insurance Premiums.
  • Options B & C are for groups of 10-50 enrolled employees, and when the employer pays either 25%-74% of premiums (B), or less than 24% or NONE of the premiums (C).
  • These plans require a minimum enrollment of 10 employees.
  • Any employee premiums can be paid with PRE-TAX income, which saves money on taxes & reduces the net cost of the insurance (must be set up properly to be able to do so).
  • There are many ways to modify these benefits, such as frequency, copayments, material allowances, network of providers, etc.
